Privacy terms must reflect the operating provider.
This page intentionally does not publish invented privacy representations. Before launch, the purchaser must replace it with a notice describing its actual entity, data practices, vendors, patient rights, security measures, contact information, and applicable law.
Required launch inputs
- Legal name, business address, privacy contact, and effective date.
- Information collected on public pages, intake forms, portals, payments, and communications.
- Purposes, legal bases where applicable, disclosures, retention, and security practices.
- Tracking technologies, analytics, advertising vendors, and consumer choices.
- HIPAA Notice of Privacy Practices when required and kept distinct from the general website notice.
- State-specific consumer-health-data and general privacy disclosures where applicable.
- Instructions for access, correction, deletion, appeals, complaints, and other applicable rights.
